Liability Waiver & Participation Agreement
By registering for this 2‑hour Adult Social Club session, I understand and agree:
• Participation is voluntary. • I accept normal risks associated with social and recreational activities. • Staff and volunteers do not provide medical care, medication administration, or personal care. • Volunteers provide general supervision only and are not trained caregivers. • I release the organizers, volunteers, and facility from liability for injury or loss, except in cases of gross negligence or intentional misconduct. • In an emergency, I authorize staff to call 911 and contact my emergency contact. I am responsible for any resulting medical costs.
